I know there has been some cataclysmic failures of cooling fans. Has Dodge come out and said there is an issue? Has anyone replaced their fan even though there was no apparent issue as a preventative maintenance measure?

I'm debating whether I want to just do this.

And yes, many have replaced them proactively as a preventative measure. This is particularly true for folks that don't live in the pre-defined 'warm weather' states listed in the notification.
I checked that and mine apparently isn't included in the SB. It was purchased (and delivered new) to MD and is a Daytona R/T but I guess it does not have the "severe duty" engine cooling. Not sure how to tell that. Is it an RPO code somewhere I can check? It isn't on the window sticker.

Are there documented cases of the fans failing on cars not covered by the SB?

It doesn't only apply to vehicles with Severe Duty engine cooling, it also applied to vehicles with the other engine cooling fans as stated in the CSN L08

NOTE: This notification applies to all of the above vehicles equipped with severe duty engine cooling (sales code NMS) or vehicles equipped with (sales code XDL or XDK) with “sold to” sales codes and/or are currently registered in warm climate states, built through April 29, 2008

NOTE: All vehicles with sales code NMS are involved in this notification regardless of vehicle location. Vehicles with sales code XDL or XDK originally sold or currently registered in the following states are also involved in this notification:

Alabama
Arizona
Arkansas
Kansas
Louisiana
Maryland
North Carolina
Oklahoma
South Carolina
California
Mississippi

Perhaps @DodgeCares can help you figure out if you vehicle should be covered. Because it isn't an actual recall, the on line check may not show it.

I wonder if my car is covered in this. I have a 2008, but I'm not sure if it was built prior to April 28th, 2008. However, the only open recall I've seen for my car is for the ignition system. If it is in the year range, do I just go to the dodge dealership and they do this service free of charge?

Edit: Car was purchased by original owner in January 2008, so it's safe to say it was built before that date. Not sure how to check the sales code either.

The TSB fan kit costs about $12-$15 at the dealer and they are fairly easy to replace.
